The narrative begins in 1761, when the young Charlotte, played by India Amarteifio, arrives in England after being selected to marry George. Their wedding is arranged for the day she meets him, and the series follows the immediate shock of royal marriage, cultural dislocation, and the pressure placed on her to produce an heir. Golda Rosheuvel portrays the older Charlotte in the Bridgerton-era storyline.

The show uses two timelines to connect Charlotte's early life with her later experiences as a queen. The contemporary timeline includes established characters from Bridgerton, including Lady Danbury, played by Adjoa Andoh, and Violet Bridgerton, portrayed by Ruth Gemmell. Their friendship develops during Charlotte's first years at court and later becomes central to the story.

Arsema Thomas plays a young Agatha Danbury, whose marriage to Lord Danbury gives her a title but little companionship. Agatha negotiates the rules of the ton while seeking independence, and her relationship with Charlotte becomes both practical and emotionally significant. The older Agatha and Violet revisit that history after Violet's husband dies, revealing how the women's public identities conceal private disagreements and long-standing choices.

George's storyline adds a historical tragedy to the romance. Corey Mylchreest plays the young king, while James Fleet appears as the older George. The series depicts his efforts to hide symptoms of a serious mental illness and his scientific interests, alongside Charlotte's struggle to understand what is happening. Although the show dramatizes these events, George III's real life included periods of severe illness, and he was known for supporting scientific institutions, including the Royal Academy.

Several major plot developments are fictionalized for the series, but the production retains recognizable historical anchors. Charlotte and George had fifteen children, and the program makes the succession question a recurring source of anxiety. The story also presents the “Great Experiment,” a proposed integration of Black peers into the British nobility. This concept connects the show's racial politics to Bridgerton's alternate history, while making clear that the characters' lives are shaped by a social transformation rather than by documented events alone. Its imagined premise is part of the show's design deliberately.
