Chaos Spewer would fit best in an aggressive Rakdos (black-red) deck that either benefits from sacrificing or weakening its own creatures, or actively wants -1/-1 counters placed on them—such as decks built around undying, persist, death triggers, or creatures that grow when they get counters (for example, in formats where cards like Hapatra, Vizier of Poisons, Nest of Scarabs, or creatures with persist are legal). The optional {2} payment makes it flexible: in slower midrange shells you can pay to avoid the drawback and keep tempo, while in synergy-driven builds you intentionally decline to trigger blight for value. However, compared to more efficient Rakdos two-drops—such as Kroxa, Bloodtithe Harvester, Dreadhorde Butcher, or even staple value creatures like Bloodtithe Harvester—Chaos Spewer is generally weaker unless your deck is specifically built to exploit -1/-1 counters; outside of that niche synergy, giving yourself counters is a real cost, making it unlikely to see competitive
